Hypertension Management Program

All patients with one or more high blood pressure readings on file OR an established diagnosis of high blood pressure (hypertension) are automatically eligible for the hypertension management program.  If you meet either of the above criteria, you may be contacted to invite you to take part in the hypertension management program.

Benefits of the program include:

  • 30 minute appointments with the Nurse Practitioner to discuss lifestyle interventions and/or medications to help manage your blood pressure.
  • Optional one-on-one appointments with a Registered Dietician
  • Free home blood pressure monitor lending program
  • Group education sessions facilitated by Registered Nurses and a Registered Dietician during heart month (February)
  • Take-home educational materials/resources to help you better understand and manage your blood pressure.

Village Family Health Team is open and seeing patients. As per By-Law, a face covering is required for everyone except children under 2. For the safety of you and our team, you will be asked screening questions before your appointment. If you require someone to attend your appointment with you, please limit to one additional person.

Toronto Public Health Flu Clinics

The flu vaccine is free for people six months of age and older who live, work or attend school in Ontario. A health card is not required at a TPH clinic. Flu clinic appointments can be booked online here  . Due to the COVID-19 pandemic, walk-in appointments will not be available this year.
