Social Worker (0.8 FTE, Part-time, Contract September 2024 to August 2025)
Village Family Health Team is dedicated to serving the vibrant and rapidly growing neighborhoods of Liberty Village, King/Queen West, and Parkdale. Position Summary:
The Social Worker will be integral in providing and supporting patient care and the delivery of FHT programs. Working collaboratively within an inter-professional healthcare team, the Social Worker will practice to the full extent of their scope as defined by the College of Social Workers and Social Service Workers of Ontario.
Key Responsibilities:
· Conduct and develop comprehensive psychosocial clinical assessments for referred patients.
· Provide short-term psychotherapy to individuals, or within a group setting
· Collaborate with inter-professional team members to initiate and manage patient care.
· Educate and promote mental health and addiction awareness to individuals, groups, families, and couples.
· Offer up-to-date information on mental health community resources and referrals to patients and the team.
· Contribute to the training and clinical placement experiences of health professional students or residents.
· Coordinate data collection and analysis for program and patient care evaluation.
Qualifications:
· Master’s Degree in Social Work.
· Current registration with OSCWSSW (membership must be in good standing)
· Demonstrate ability provide evidence-based therapy such as CBT, DBT, EFT, CPT, CBTi, and/or IPT is an asset.
· Comfort working with individuals with trauma histories in a short-term model is an asset.
· Knowledge of harm reduction principles and their application.
· Experience working with diverse gender, sex, cultural, and racial backgrounds.
· Demonstrated clinical leadership, advanced knowledge, and decision-making skills.
· Excellent communication and interpersonal abilities.
· Proficiency in computer use, particularly with Electronic Medical Records.
Reporting Structure:
The Social Worker will report to the Executive Director for day-to-day operations.
Job Specifications:
Location: 171 East Liberty St., Unit 102.
Hours of Work: 30 hours per week, generally aligning with FHT practice operations, with some evening clinics.
Status: 0.8 FTE, contract position from September 2024 until August 2025.
