Hours of work: 37.5 hours per week, Monday – Friday (9 am – 5 pm)

Position Overview: We are seeking a dedicated Registered Nurse to join our dynamic inter-professional team. As a member of our team at VFHT, you will play a pivotal role in delivering high-quality care to our patients. You will work closely with other healthcare professionals (physicians, psychiatrists, nurse practitioner, social workers, nurses, chiropodist, pharmacist, dietician, physiotherapist, and administrative team) to assess, plan, and provide comprehensive nursing care to meet the needs of our patient population. You will practice within the full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.

Responsibilities:

  • Conduct thorough patient assessments, including health history, physical examinations, and medication reviews.

  • Coordinate appropriate diagnostic tests and procedures in consultation with family physicians or in accordance with medical directives. This includes performing Well Women Exams, monitoring anticoagulation therapy, STI screening and other acute medical treatments.

  • Implement various goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and inter-disciplinary team.

  • Apply self-management and other strategies to support the development of an informed activated patient.

  • Help facilitate the incorporation of proven strategies for individual family physician practices to provide more effective care delivery.

  • Participate as a clinical team member in the development and delivery of targeted programs including but not limited to chronic disease management, child and family health, illness prevention and screening.

  • Provide timely and informative reports as directed and requested by the Executive Director and/or Physician Lead.

  • Actively participate in staff meetings, to exchange ideas, receive mutual support and monitor program outcomes as well as present case studies.

  • Administer medications and treatments as prescribed, while coordinating with the team to optimize patient outcomes.

  • Collaborate with our community partner organizations to deliver holistic healthcare needs of our patients.

  • Maintain accurate and up-to-date electronic health records (EHR) in compliance with regulatory standards.

Qualifications and Requirements:

  • Bachelor of Science in Nursing (BScN) required. Current registration, in good standing with no practice restrictions as a Registered Nurse (RN) with the College of Nurses of Ontario.

  • Current BCLS required.

  • Previous experience in a primary care or family medicine setting preferred.

  • Strong clinical assessment and critical thinking skills with a strong knowledge of primary health care, mental health care, and social determinants of health.

  • Strong understanding and utilization of evidence-based practice.

  • Excellent communication, interpersonal, and time management skills.

  • Ability to work both independently and collaboratively in a team-based care model.
