Job Title: Clinical Pharmacist (0.4 FTE – 12-month temporary part-time contract)

Position Summary

·       0.4 FTE position covering a 12-month leave beginning June 2025

·       Conduct comprehensive medication assessments/reviews including medication history and medication reconciliations for individual patients

·       Identify, prevent, and resolve actual and potential drug therapy problems

·       Formulate comprehensive care plans which incorporate individual patient preferences and goals while collaborating with interdisciplinary team

·       Act as a clinical pharmacy resource for internal and external health care professionals in a patient-focused practice

·       Work within an interdisciplinary team to promote rational evidence-based prescribing to optimize patient outcomes

·       Liaise with community and hospital pharmacists to facilitate patient care

·       Provide clinical direction on complex cases

·       Establish initiatives that increase patient safety

·       Participate in the development of evidence-based practice treatment guidelines

·       Participate in the implementation of the guidelines including:

o   Education

o   Audit and feedback on usage

o   Feedback on patient risk reduction and outcomes as appropriate

·       Participate and leads quality improvement initiatives with the goal of improving patient safety

·       Medication training and device demonstration

·       Provide Drug Information:

o   Patient medication questions

o   Clinician questions relating to drug interactions, medication tapers, pharmacotherapy recommendations, renal dosage adjustments, medication costs and coverage

·       Involvement in VFHT programs:

o   Metabolic Program

o   COPD Program

o   Anticoagulation/INR Management

o   STOP Program

JOB REQUIREMENTS:

Skills

·        Ability to work independently

·        Strong written communication

·        Strong verbal communication skills

·       Time-management and organizational skills to manage varied tasks without direct supervision

·        Experience with Accuro an asset

Education/Experience

·       Completion of a PharmD or Bachelor of Science Degree in Pharmacy from an accredited program of study required

·       Minimum 3 years experience in community-based care an asset

·       Experience or residency in primary care an asset

Qualifications

·       Current registration in Part A of the register with the Ontario College of Pharmacists required

·       Pharmacy practice in related areas of patient-focused or population health

·       Certified Diabetes Educator (CDE) preferred

·       CAMH Teach Certification an asset
