MEDICATION RENEWAL POLICY

To help us renew your prescription on time, please remember:

  1. Review your medication needs at each appointment.
  2. Bring your medications to each appointment with your Doctor or Nurse Practitioner.
  3. Check the amount of your medications between appointments to make sure you have enough.
  4. If you run out of medications before your next appointment:
  • Call your pharmacy and have them fax your renewal to us on 416 599 2001
  • Allow a minimum of five business days for us to respond to your pharmacy
  • Do not call the village Family Health Team for renewal requests.

Village FHT continues to rigorously screen for COVID-19. People with any of the following symptoms must have a negative PCR test in the last 72 hours before coming into our clinic.

  • Fever or chills
  • Cough
  • Trouble breathing
  • Decrease or loss of taste or smell
  • Nausea, vomiting or diarrhea (< 18years)
  • Very tired, sore muscles or joints (18+ years)
