MEDICATION RENEWAL POLICY

To help us renew your prescription on time, please remember:

  1. Review your medication needs at each appointment.
  2. Bring your medications to each appointment with your Doctor or Nurse Practitioner.
  3. Check the amount of your medications between appointments to make sure you have enough.
  4. If you run out of medications before your next appointment:
  • Call your pharmacy and have them fax your renewal to us on 416 599 2001
  • Allow a minimum of five business days for us to respond to your pharmacy
  • Do not call the village Family Health Team for renewal requests.

Village Family Health Team is open and seeing patients. As per By-Law, a face covering is required for everyone except children under 2. For the safety of you and our team, you will be asked screening questions before your appointment. If you require someone to attend your appointment with you, please limit to one additional person.
